Ontario Lacrosse Hall of Fame Nomination Form

There are five sections to this form:


Section 1: Nominator's & Nominee's contact information

Section 2: Nominee's Qualifications - Player

Section 3: Nominee's Qualifications - Builder

Section 4: Nominee's Qualifications - Official

Section 5: Upload Picture of Nominee & any supporting documents. Please be prepared to upload a colour head & shoulders photo of the Nominee (passport grade 300DPI minimum) and any supporting documents in the final section of this form.


Please click "Submit" once you have completed adding all of the nomination information. You will receive an acknowledgement email within 24 hours of submitting your nomination form. Deadline is April 15th of current year for nominations. All nominations that meet the qualifying criteria will be placed in the Library for a minimum of one year before they can be considered for induction. Please list all special awards and recognition e.g. MVP, Top Scorer, Team Captain

Include any other pertinent information about this person as a 'Player" of lacrosse.
