Cooperator Questionnaire
This form is intended for the collection of data pertaining to forest management or similar projects in order to give Conservation District planners a better understanding of the resources on the property and the landowner’s intentions for their property. This information will allow planners to better incorporate the landowner’s intentions with the recommended management practices while also accounting for current best stewardship practices, professional knowledge, and legal requirements. Conservation Districts are non-regulatory entities and as such none of the information provided will be shared with regulatory entities.

Is there one or more homesites on the property and/or do you currently live on the property?
You would likely have an Farm Service Agency farm number if you've previously done cost-share work on the property or applied for an easement program through the USDA or other federal agencies.
Nearest named river or stream is helpful if watershed is unknown.

Please list any goals or objectives for your property. This can include habitat improvement, timber harvest, invasive species control, maintaining or improving property appearance, etc.
Please list any historic information you have about the property, including when you purchased it, what management activities have taken place on the property either prior to your purchase of the land or that you have done.
Please list any current concerns for forest health on you property, such as if there are any signs of die-off or disease, lack of species diversity, forest is too dense or not dense enough, etc.
Do you have any concerns about excessive fuels on your property?
Are you interested in creating a fire-resistant home?
Are there major infestations of invasive species? If so, what species?
Do you have issues with soil compaction or erosion?
Do you have any water resources on your property (wells, streams, springs, ponds, etc.)?
Please describe. Are they perennial or seasonal, fish-bearing or non-fish-bearing, domestic use, etc.
Are there any issues with access to the property, including access to certain areas of the property internally? If so, please describe.
Do you have any culverts or other crossings which are undersized or in disrepair? If so, please describe.
Would you like information about road or trail creation and maintenance?
Do you have any issues with wildlife damage on the property? If so, please describe.
Are you interested in maintaining or improving wildlife habitat?
Are there any specific species you would be interested in seeing more of on your property?

Is climate resilience important to your forest planning?
Are you interested in carbon sequestration?
Are you interested in pursuing carbon credits if that is an option for private landowners in the future?
Do you have a plan in place for the future ownership and care of your property?
Would you be interested in receiving help with legacy planning or discussing the types of options available, such as how to get family involved, land donation, perpetual easements, etc.?
