2025 UI Health's Award Season Nomination

Complete this nomination form, including necessary email addresses.
Upload a minimum of two (2) but no more than five (5) detailed letters outlining why your nominee is deserving of their award. Letters should address the criteria found below and support UIH's Mission Driven as One! If possible, please combine the letters into a single PDF file for submission.

Award Selection

Physician/APP/Resident/Fellow of the Year - Nominate a physician, resident/fellow or advance practice provider that you feel demonstrated the qualities of a true class act. One attending physician, one APP, and one resident/fellow will be chosen!

Joseph V. Abraham Leadership - This award is given annually to one hospital employee in Imaging and Diagnostic Services (e.g., Radiology, Cardiovascular Services, Endoscopy Lab, Radiation Oncology, and Short Stay Unit). The recipient will be an employee who demonstrates excellence in leadership and who have at least three and not more than ten years of experience in the Unit.

UIC Award of Merit - This award was established to provide a mechanism for campus leadership to recognize and honor outstanding Civil Service and Academic Professional employees. In 2025, 10 awards will be presented to UI Health Employees. Recipients will receive a UIC Award of Merit memento and a $3,000 monetary award. All recipients will be recognized at the 2025 Employee Recognition Award Ceremony and Reception which will be held in November location details will follow in future communications.

Criteria for award winners:

• Puts the patient first in all encounters and consistently contributes to the delivery of exceptional patient care and experience.

• Displays UI Health’s Mission-Driven As One ethos, aligning their purpose and actions in support of achieving our long-term goals.

• Collaborates with teams across all the health enterprise in support of delivering exceptional patient care, experience, and service.

• Emphasizes quality improvement, innovation, and responsible resource use to elevate care and increase efficiencies.

• Fosters an environment that encourages respect, integrity, fairness, and diversity; recognizes others for their contributions to UI Health.
