Domestic Student Travel Registration

for Individual Travel

The following information is required for each student participating in a trip related to the University per OP 91.305 DOMESTIC TRAVEL BY STUDENTS. This information will only need to be provided once per year, and it will only be used in an emergency during your travel. By filling out this information, you agree to receive notifications in the event of an emergency. Notifications will inculde critical updates or important safety information. Your contact information will not be shared for any other purpose. Standard message and data rates may apply.


Step 1

The traveler must register each trip using this form.


Step 2 (Optional) for Multi-Location (city) Trips

Students should submit a form entry for each location (city) if the trip involves multiple locations (cities).


Example:

If you are visiting and participating in activities in Corinth, MS, and then Tupelo, MS, on a single day or over multiple days, two trip registrations must be entered.
