St. Thomas Aquinas Faith Formation Registration

Dear Families,

This form is for both NEW and RETURNING Students to Faith Formation Classes. All Students and their families should register every year!

Please complete this form as completely as possible to ensure that your family is enrolled in the correct classes in August.

Please also complete your payments at the bottom of this form. Please note: Faith Formation and Sacramental classes are paid separately.

Estamos emocionados de comenzar nuestro nuevo año de Formación en la Fe. A medida que continuemos creciendo como comunidad de fe, estaremos ayudando a las familias a crecer como Discípulos Misioneros.

Este formulario es para los estudiantes NUEVOS y los QUE REGRESAN a las Clases de Fomación en la Fe. ¡Todos los estudiantes y sus familias deben registrarse cada año!

Por favor complete este formulario lo mejor posible para asegurarnos que su familia esté inscrita en las clases correctas y obtener toda la información necesaria. Anunciaremos las fechas de formación en la fe tan pronto como podamos. Esperamos comenzar las clases en agosto.
