Rolling Application for FAN Grant Program

The FAN grant program gives 100% of our funding to community centered programs that partner with parents and caregivers. We prioritize programs that are led by and/or work for BIPOC and/or immigrant communities.

We hope that this application is easy and quick to fill out. If you prefer not to fill out the form, but to apply by having a brief conversation with someone on our team, please contact Lisa Foss at lfoss@cooperhouse.org.

If you have any questions, please email Lisa Foss at lfoss@cooperhouse.org.

What is the FAN?

•    FAN (Facilitating Attuned Interactions) is a framework for building attuned, collaborative relationships between human service professionals and the families they serve.

•    The FAN was designed specifically with the prenatal to five work force in mind.

Practitioner FAN is for teams of providers who support caregivers of young children, along with the supervisors of those providers. The training is most impactful when you attend as a team, so you can support each other in integrating the FAN concepts into your work.

Supervisor FAN is just for supervisors. It focuses on strengthening the supervisor-supervisee relationship, resulting in supervisors and supervisees who are more reflective and collaborative, as well as supervisors who are more engaged and less stressed.

Supplemental FAN Trainings include FAN sessions that review and deepen your understanding and application of the FAN to your work.

  • If you apply for Practitioner or Supervisor FAN in the grant program and you are admitted, all future Supplemental FAN trainings will be available to you at no cost.
  • If you have already taken a FAN training and you want your team to be able to attend future Supplemental FAN Trainings at no cost, this option is for you.

If you are applying for a single program of a larger organization, please provide your program's name.

Please use this question to indicate how many people from your organization will attend if your program is accepted. If you will attend, include yourself in this number.

If anyone on your team is a supervisor who carries a caseload, include them here.

Please include any home visitors, early interventionists, or other providers who work directly with caregivers/families. If you are applying for Supervisor FAN, please enter "0."

Check all that apply.

Please check to see if you are currently funded by Washington State's Home Visiting Services Account

Approximately what percent of your program's clients are people of color and/or immigrants?*
Approximately what percent of your program's clients are from rural areas?*

By "rural areas" we mean areas that are not part of a big city. In other words, they have few people living in the area, limited access to large cities, and considerable traveling distances to areas for work and everyday-living activities.

We encourage you to limit your answer to 200 words or less.

The FAN Grant prioritizes programs with the least access to trainings/professional development.

Please provide us with information about your program's access to trainings, professional development, and reflective consultation.

Does your program pay for staff trainings or other professional development?*

This is optional.

Would the trainees prefer an in-person or virtual training?*

The in-person training would take place over two consecutive days, from 9-4 each day.

The virtual training would take place over four half-days, for 3 hours, 15 minutes each day.

If your team needed to travel to the training location, Cooper House would cover your travel costs (gas, hotel if needed, meals on travel days)
