Annual Laboratory Registration

If you occupy a laboratory, studio, or clinic where chemicals/biologicals or other hazardous materials are used, you are required to complete this form prior to November 1st of each year.


Laboratory Registration- should be completed annually and/or upon assignment of lab, studio or clinic space.


Please fill out a separate form for each laboratory. This will help EHS maintain an accurate count of these spaces.

Requestors will not be able to submit hazardous waste removal requests to have wastes removed from laboratories until the laboratory has been properly registered with EHS and provided a current chemical inventory for the lab.

Please answer the following questions as YES or NO and list materials that will be used. Attach an inventory report to the file upload at the end of this form, if necessary.

By checking this box, I confirm that my laboratory does not contain any chemicals or bioagents.


You are still required to answer all the questions.




Chemical

Will you be using/storing hazardous materials?*
