This is for both FORSEEABLE and UNFORSEEABLE occurrences.
FORSEEABLE-(If the need to use sick and safe leave is foreseeable (for example, a scheduled doctor’s appointment), the employee must provide notice 7 days prior to leave use by complying with and completing the procedural requirements for requesting or reporting use of earned sick and safe leave.) If the leave is for more than two consecutive shifts (i.e., three or more workdays in a row), your supervisor may request that you provide verification that the leave use was appropriate by uploading acceptable documentation.
UNFORSEEABLE-Please also provide a brief explanation, in the space below, as to why this leave was not foreseeable.
