Completion of this form is required to receive a CRN# to enroll in UWP 92 or UWP 192 units. The Professional Writing Internship Coordinator will review your request and, upon approval, send you an email that includes a CRN# to enroll in units.

Please indicate on the form how many units you are requesting. One unit of credit requires 30 hours of writing-related work during the quarter; four units require 120 hours.
