NCCARE360 Community Survey

The following survey was developed to help identify organizational, programmatic, and training needs for the statewide implementation of NCCARE360. As the first statewide network that unites health care and human service organizations using a shared technology that enables a coordinated, community-oriented, person-centered approach for delivering care, community partners are committed to identifying barriers that prevent organizations’ ability to utilize the network. Your feedback will be used by NCCARE360 administrators and implementation partners to strategically plan for opportunities to support community-based organizations that currently use and are interesting in using the NCCARE360 network to identify, address, and provide resources for unmet social needs in North Carolina. Please complete this survey by Friday, October 15 at 5:00 pm.
