Instructions

1. Read these procedures.

2. Deposit or transfer (via online bank transfer, Gcash, or other e-wallet) your payment to CEM BPI account.

Cost: Php50.00 for every copy plus P80.00 shipping fee (Courier service only, no pick-up arrangement)

Example: 1 copy - P130.00; 2 copies - P180.00; 3 copies - P230.00

IMPORTANT Notes:

The bank may charge services fees and inter-bank transaction fees ON TOP of the ERF fees. Inquire with your bank and to pay for such fees in addition to the ERF fees.

Bank OTC payments for NMAT ERF copies do not require a reference number. Deposit ONLY into the specified bank account; deposits made to other accounts will not be processed or honored.

Online banking transactions will be accepted. Please ensure that the proof of payment indicates the transaction was SUCCESSFULLY completed and includes the recipient's account number, amount, transaction date, and transaction/reference number.

The attachment indicating that the 'transfer is being processed' is NOT a valid proof of payment.

For reshipping request only, only the shipping fee is required. It will be processed once the actual document has been returned to us. However, please note that unclaimed RTS ERFs are automatically disposed of two (2) years after their receipt by CEM. In case, you will need to also request for another copy of your ERF.

3. Accomplish and submit the Request for NMAT Examinee Report Form (ERF) Copy. All fields are required.

4. Check your Inbox, SPAM, or other email folders for status updates on your request. After information and payment have been validated and email confirmation is received, allow 10-15 business days for the shipment of package.

Please note that the NMAT Secretariat will not process any duplicate copy requests for the recent examination date until after original copies are distributed.
