Faculty Misconduct Authorization and Disclosure Form

We accept as our operational standard that the university should be a place where people can work and learn together in a safe environment, free of violence, harassment, discrimination, exploitation or intimidation.
Following models at other universities, Ohio State requests all candidates for tenured positions to disclose any findings of misconduct against them or ongoing investigations or disciplinary proceedings and to provide written authorization giving Ohio State permission to seek certain information from current and past employers.
All individuals submitting an application for a tenured position (tenured associate professor or tenured professor) must complete the Faculty Misconduct Authorization and Disclosure Form as part of their application process.
The authorization and disclosure form requires candidates to disclose any past findings of misconduct as well as current disciplinary proceedings and any ongoing investigations against them, and further to execute an authorization permitting the applicant’s current and former employer(s) to share information with Ohio State about final findings of employment-related misconduct or current disciplinary proceedings against the applicant and/or any ongoing investigations related to misconduct.
Any candidate who fails to fully complete the disclosure and authorization form or otherwise fails to cooperate with this process will not be eligible for the position.
If nothing is disclosed related to any past findings of misconduct or ongoing disciplinary proceedings or investigations, no action will be taken.
If there is disclosure of any findings, disciplinary proceedings or ongoing investigations, the Office of Academic Affairs (OAA) will review the information provided and may further contact the candidate’s current and past employers to gather additional information as needed in accordance with the authorization.
OAA will convene a meeting with the unit head (e.g., department chair, school director), dean, the Office of Legal Affairs and the Office of Faculty Affairs to discuss the information provided and determine next steps.
Should information arise following hire that a candidate failed to meet this duty, that individual will be subject to immediate disciplinary action, which may include discipline up to and including termination of employment.
