Please note: Wests Future Stars are to complete the Monthly Engagement Form specific for their program as opposed to this form.

The purpose of this Results and Performance Update form is to Provide the Academy with updates on your results and performance, whilst an Academy athlete with the intent to;

• Provide results and performance updates for Academy Staff to interact with athletes and their coach and support staff.
• Provide results and performance information, including images that can be used in media releases / social media stories to promote the athlete and the respective sport program.
• Provide additional feedback on topics / education for inclusion within the program.

This form can be completed at any time whilst the athlete is a part of their respective sport program.

Athletes are reminded that they should use the Athlete Journal / XPS to record information during the month in the lead up to completion of this form.

And are encouraged to reach out to sport@swsas.org.au at any time should they wish to discuss aspects of the program with either Academy staff, or their program coach and support staff.
