Prior to starting this form: Complete your UNM Tuition Remission paperwork, remember proper signatures are REQUIRED (https://app.box.com/file/797023179707?s=u9389zmisaw0dugxyfla4x3ygqy2zavs), complete your UNM PR and make a copy of your check, you will be required to upload a copy of these documents at the end of this form.


Registration: Due to a limited enrollment of 60 participants, advanced registration is encouraged. All registrations received on or before July 25, 2025 will be confirmed by email, and it will include a confirmation letter, a map showing the conference location and parking. If you have not received your confirmation letter, you may contact UNM CPL at 505-272-3942 to confirm your registration. Please confirm your registration prior to making your travel arrangements.

Online Course Access: Registrants will be provided an email with a link to the Online Portion of the ALSO Provider Course. Content consists of a pre-test; recorded lectures; online case studies; recorded workstation demonstrations; post-test; and course evaluation. Upon completion of the evaluation, the learners must CLAIM CREDIT to show as “Complete” in the Sponsor’s (UNM CPL) Course Dashboard and to qualify to attend the Live Course. The Online Portion of the course must be completed no later than August 4, 2023. You will be able to download the presentation content and chapters for further review, printing or future reference. Please note: You also need to visit the “Optional Workstations” area in the online portion and review/download those slides prior to the course.


Cancellation: If you pre-register and cannot attend, UNM CPL will refund tuition, less $190.00 ($40.00 administrative fee and $150 Online Access fee), provided it is received in writing on or before July 25, 2025. No refunds will be issued after this date or for non-attendance. Courses are subject to cancellation. In the event this course is cancelled, UNM CPL is not responsible for any airfare, hotel or other costs incurred by participants.

Breakout Session

Each participant will attend (1) Optional Workshop, please select your 1st and 2nd options below. Minimum and maximum numbers have been established for these breakouts.


3rd & 4th Degree Lacerations is tentative based on demand, if there is not enough demand you will be placed in your 2nd option. This session is available ONLY to experienced physicians and CNMs that deliver babies. NO RN’s or medical students are allowed to take the 3rd & 4th degree workshop.


OB Residents - Can select 1st & 2nd or Birth Crisis (Not eligible for 3rd & 4th, will take Ultrasound at residency school)
