OFFICIAL CONTEST RULES
AFFINITY FEDERAL CREDIT UNION (“the Credit Union”) “Affinity Members of the Year 2023” CONTEST OFFICIAL RULES NO PURCHASE, OBLIGATION OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.
ELIGIBILITY: “Affinity Members of the Year 2023 Nomination” Contest (the “Contest”) is open to legal residents of the states of New York, New Jersey and Connecticut. Void in all other locations (including but not limited to AZ, FL, RI, TX, and Puerto Rico), and where prohibited or restricted by law. Entrants must be 18 years of age or older and an Affinity member at the time of entry in the Contest. Employees, officers, and directors and their immediate family members including spouse, parent, child and sibling and their respective spouses, regardless of where they reside, and members of the same household, whether or not related to the Credit Union and their respective parents, affiliated companies, subsidiaries, licensees, members distributors, dealers, retailers, printers and advertising and promotion agencies, and any and all other companies associated with the Contest are not eligible to participate or win a prize. Contest is subject to all applicable federal, state and local laws, rules and regulations.
HOW TO ENTER: The Contest begins at 11:59:59AM EST on 11/1/2023 and ends at 11:59:59PM EST on 12/8/2023 (“Entry Period”). During the Entry Period, there is one (1) way to enter: via Online Method. No other method of entry will be accepted. Enter to win Affinity’s Members of the Year and benefit your favorite charitable cause. Limit one (1) Affinity member nomination per person, per e-mail address and per household, regardless of method of entry. In the event of a dispute as to the identity or eligibility of a Winner based on an e-mail address, the winning entry will be declared made by the “Authorized Account Holder” of the e-mail address submitted at the time of entry provided he/she is eligible according to these Official Rules. The “Authorized Account Holder” is defined as the natural person to whom the applicable Internet service provider or other organization (such as a business or educational institution) has assigned the e-mail address for the domain associated with the submitted e-mail address. The Credit Union reserves the right to modify these Official Rules for clarification purposes without materially affecting the terms and conditions of the Contest.
Online Method: During the Entry Period, go to www.https://app.smartsheet.com/b/form/e5d9fbd569704e679cebb5abfe3ab8a9.com and complete the form in its entirety. By submitting your completed online entry form as directed, you (or the member you have nominated) will receive one (1) entry in the Contest. Limit of one (1) entry per person and per email address. Entrants must fully complete and submit all non-optional data requested on the online entry form to be eligible. Incomplete entries are void.
DRAWING: There will be three (3) winners of the Contest, who will be selected based on evaluation of their nominations and completed activities. Winners will need to have demonstrated a firm sense of social responsibility and giving back to communities. One Affinity member winner will be awarded per region in our branch footprint (New Jersey, New York, and Connecticut).
The participants who are the winners of the Contest will be notified on or about 12/15/2023. Winner need not be present to win. Limit one (1) prize per person, family and per household.
PRIZES: Two 2 prizes are available to be awarded to each of the three (3) winners. Winners will each receive a $500 credit into their Affinity savings account (total retail value $500). The winning member must have a savings account with Affinity within 60 days of winning to receive the $500 credit. Winners are responsible for all applicable federal, state and local taxes and fees associated with the prize receipt and/or use. In addition, for every winner, a contribution from the Affinity Foundation will be made to an organization that aligns with the selected pillar, choosing from: Disaster Relief, Education, Financial Wellbeing, or Healthcare. A donation from the Affinity Foundation for $500 will be made to an organization that aligns with the selected pillar on behalf of each Member of the Year by 12.31.2023. The Affinity Foundation is a 501(c)(3) nonprofit organization whose mission is to provide Affinity's members and our communities with solutions to improve financial wellness, educational programs that enable self-sufficiency, and support that ensures access to essential healthcare services (EIN #20-2706810).
NOTIFICATION: The potential Winners will be notified by e-mail, mail and/or telephone on or about 12/15/2023 at the sole discretion of the Credit Union. In the event that any potential Winner does not respond to any such notification within seven (7) days of issuance date, a disqualification will result, the Prize will be forfeited and, at the Credit Union’s sole discretion and time permitting, an alternate potential Winner may be chosen from among all remaining eligible entries. The potential Winners will be required to execute, have notarized and return an Affidavit of Eligibility and Release of Liability and, unless prohibited by law, Release of Publicity, within seven (7) days of issuance date. If all required documents are not properly executed and returned within the specified period of time a disqualification will result, the Prize will be forfeited, and any alternate potential Winner(s) may be randomly selected from among all remaining eligible entries. Refusal or return of such documents as non-deliverable or potential Winner’s noncompliance with these Official Rules will also result in disqualification.
WAIVER OF LIABILITY/PUBLICITY RELEASE: By participating in the “Affinity Members of the Year 2023 Nomination” contest and submitting an entry, each entrant agrees to be bound by these Official Rules. All entry requirements waive and release any and all claims against the Credit Union each of their respective parents, affiliated companies, subsidiaries, licensees, distributors, dealers, retailers, printers, representatives and advertising and promotion agencies, and any and all other companies associated with the Contest, and all of their respective officers, members, directors, employees, agents, volunteers and representatives (collectively, “Released Parties”). The Winners, by acceptance of Prize, grants to the Credit Union, each of their respective designees the right to publicize such Winner’s name, address (city and state of residence), photograph, voice and/or other likeness and prize information in any and all media now known or hereafter devised, throughout the world, in perpetuity, without additional compensation or consideration, notification or permission, unless prohibited by law.
GENERAL CONDITIONS: Released Parties are not responsible for stolen, late, incomplete, illegible, misdirected, lost, damaged, delayed, undelivered, inaccurate, or garbled entries. Entries generated by a script, macro or other mechanical or automated means or by any means which subvert the entry process will be disqualified. Released Parties are not responsible for lost, interrupted or unavailable network, server, Internet Service Provider (ISP), website, or other connections, availability or accessibility or miscommunications or failed computer, satellite, telephone or cable transmissions, lines, or technical failure or jumbled, scrambled, delayed, or misdirected transmissions or computer hardware or software malfunctions, failures or difficulties, or other errors or difficulties of any kind whether human, mechanical, electronic, computer, network, typographical, printing or otherwise relating to or in connection with the Contest, including, without limitation, errors or difficulties which may occur in connection with the administration of the Contest, the processing of entries, the announcement of the Prizes or in any Contest -related materials, or the cancellation or postponement of the event. Released Parties are also not responsible for any incorrect or inaccurate information, whether caused by website users, tampering, hacking, or by any equipment or programming associated with or utilized in the Contest. Released Parties are not responsible for injury or damage to participants' or to any other person's computer related to or resulting from participating in this Contest or downloading materials from or use of the website. Persons who tamper with or abuse any aspect of the Contest or website or attempt to undermine the legitimate operation of the Contest by cheating, deception or other unfair playing practices, or intend to annoy, abuse, threaten or harass any other entrant or the Credit Union’s representatives or who are in violation of these Official Rules, as solely determined by the Credit Union, will be disqualified and all associated entries will be void. Any attempt to deliberately damage the content or operation of this Contest is unlawful and subject to legal action by the Credit Union and/or their respective agents. The Credit Union shall have the sole right to disqualify any entrant for violation of these Official Rules or any applicable laws relating to the Contest, and to resolve all disputes in their sole discretion. Released Parties (i) make no warranty, guaranty or representation of any kind concerning any Prize, and (ii) disclaim any implied warranty. All disputes will be resolved under applicable state and /or federal laws in the state of New Jersey without giving effect to choice of law or conflicted law principles (whether of the State of New Jersey, or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the state of New Jersey. The Credit Union’s failure to enforce any terms of these Official Rules shall not constitute a waiver of that provision.
CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE CONTENT OR LEGITIMATE OPERATION OF THIS CONTEST IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, THE CREDIT UNION WILL DISQUALIFY ANY SUCH ENTRANT AND THE CREDIT UNION RESERVES THE RIGHT TO SEEK DAMAGES (INCLUDING ATTORNEYS’ FEES) AND OTHER REMEDIES FROM ANY SUCH ENTRANT TO THE FULLEST EXTENT PERMITTED BY LAW.
The Credit Union reserves the right to cancel, suspend or modify the Contest if any problem corrupts the administration, security or proper play of this promotion. The Credit Union reserves the right to substitute any prize of equal or greater value if the advertised prize is unavailable. In case of multiple entries received from any person or e-mail address or household, only the first entry received from such person, e-mail address or household will be considered. Winner’s name requests must be received by 12/31/2023.
