Certificate of Approval
This form (aka the "Signature Page") is required for all School of Medicine graduate students enrolled in a PhD or master's thesis degree program. It must be approved by the OEC and the Associate Dean before, or at the time, a student submits their dissertation or thesis to the library and no later than the last day of the term graduating.
Prior to submission, students must have submitted: 1) Advancement to PhD Candidacy (PhD only) 2) Request for Advisory Committee, 3) Request for Oral Examination 4) Oral Exam Certificate. Please note that students should not complete this form until all dissertation or thesis edits have been completed (post oral examination) in consultation with their committee chair. Once successfully routed, the student and program representative will receive confirmation via email.
