To register as a Special Olympics Florida athlete, please complete the U.S. Athlete Registration Form and Waivers, Releases and Policies FormThis form must be submitted every three years for an athlete to be eligible to participate in Special Olympics.

Registration Renewal Form may be submitted annually if imedical info changes.

Unified Partners are required to complete the Unified Partner registration form and Protective Behaviors training if over age 14.

Volunteers are required to complete the Class A volunteer registration form and Protective Behaviors training.
