ASU Alcohol Permit Request

An ASU alcohol permit is required for events with alcohol held on ASU owned or leased premises that do not have a Series 5 liquor license.


Events held on third party properties do not require an ASU alcohol permit and the event host must comply with the third-party's procedures and requirements.

This request must be submitted 15 business days prior to the event date.

If a Special Event Liquor License (Series 15) is required, the event host must obtain the series 15 before submitting this request.

  2. Certificate of Insurance for the vendor serving alcohol.
  3. The name and email address for the ASU dean, director, or designee who has authority to sign off on this event.

You indicated that this event will be held on a premise that holds a liquor license. An ASU alcohol permit is not needed for events held on licensed premises.

STOP: SPECIAL EVENT LIQUOR LICENSE NEEDED

You indicated that this event will either be open to the public, include fees, or have liquor donated by a liquor distributor. These types of events must be held on a licensed premise or you must obtain a special event liquor license from the city and Arizona Department of Liquor (DLLC).


You must start this process 90-120 days in advance of your event date.

Before you can submit this request, you will need written approval from the MU. Please contact MUreservations@asu.edu and provide them with the details of the event.


Once approved, you may return to this form to submit the permit request. You will be required to attach the MU's approval in the attachment section below.

SIGNATURES AND AFFIDAVITS


If your unit is part of the Knowledge Enterprise (KE), please list Adriana.Kuiper@asu.edu as your approver.

This is the ASU Dean, Director, or other designee that has authority to sign off on this event. This individual will receive an email to review and provide approval for this request.


Entering yourself or others who are not a Dean, Director, or delegate, will result in significant delays in processing your request.


Please check and copy the email directly form Outlook before pasting here, to ensure no misspellings or errors.


ATTACHMENTS


Failure to attach any required documents will result in a delay in processing your request.


  • Certificate of insurance for the vendor serving alcohol, if there isn't a valid COI on file.
  • Special Event Liquor License (if applicable).
  • Memorial Union approval (if applicable).

If your vendor is not listed or the COI has expired you must attach tot his request a valid COI that meets ASU's insurance requirements.


Failure to attach a valid COI will result in a denied request.


NEXT STEPS

  1. The request will be reviewed for compliance. If there are any questions, the submitter will be contacted via email.
  2. The permit will be sent to the delegate who has the authority to approve this event.
  3. The ASU Police Chief will provide final approval.
  4. The submitter will receive a final alcohol permit by email.
