Early Childhood Remote Learning and Remote Services Request Form

Parents of any child who receives early childhood services may voluntarily elect for their child to participate in remote learning / remote services during the 2020-2021 school year if LPS is conducting classes under yellow or orange on the risk dial. If the risk dial is in green, only students with an individualized plan will be allowed to participate in remote learning. Please keep in mind that just like our Standard Response Protocols, every LPS building or program which provides services may not always be in the same risk color category. Submission of this form is required to request that your child’s early childhood services be provided remotely until such time that you notify the Department of Early Childhood that services resume in person. Please submit a separate form for any child you wish to receive early childhood services remotely rather than in person Once we receive your request, we will contact you for next steps. If your child and/or family circumstances change you may contact the Department of Early Childhood (402-436-1995) to enroll in remote learning . This survey is intended as a signup for families who wish for their child’s early childhood services to be provided remotely. Receiving Early Childhood Services Remotely Means: * You will attend scheduled appointments with your child and the early childhood provider(s), * For children attending LPS preschool, remote learning will occur 1-2 times each week in sessions led by your child’s Family Educator. Attendance will be taken by the teacher as if the student were physically present. Preschoolers who receive early childhood special education services will also have scheduled visits for the child and family with each of the child’s special education providers in the same frequency as included in the child’s current IEP or IFSP. Please note: While attending remote learning sessions for preschool, you must continue to notify the office of any absences (illness, activity preventing attendance, etc.) for your preschooler, * For children and families receiving early intervention services, those services will be conducted through scheduled meetings held remotely with your family’s Services Coordinator and primary service provider as outlined on the current IFSP, * For children who receive early childhood special education services in the home or community based settings (3-5 year olds) those services will be provided in the same frequency and location included on your child’s IEP, * For children and families receiving services in the Student Parent Program, those services will be conducted through scheduled sessions with your child’s Student Child Learning Center teacher or Student Parent Advocate, * It is not permissible for the parent/guardian to record lessons, take screenshots of lessons, or post information from the remote learning/services on social media concerning the remote learning / remote services, * While we work to make remote learning and remote services high quality experiences, it is not possible for all aspects of these remote services to exactly replicate face-to-face learning and services. Family (adult) engagement during remote learning is the responsibility of the parent. * Under remote learning certain state assessments or evaluations may require the family to bring the child to school for administration of the assessment or evaluation.
