MOC Part IV Project Application
Participation in the ABMS Portfolio Program recognizes activities designed to improve provider practices and the care of patients. Activities can be large in scale to address systemic and clinical issues or smaller activities to improve a specific area of practice. Activities typically involve a team of individuals although individual efforts can qualify.
Project Approval Process:
- Project lead completes Application Form and submits A3.
- Initial review is completed by CPD MOC Part IV Program and feedback provided to project lead.
- Project lead presents project to MOC Part IV Review Committee.
- Review Committee determines one of the following:
- Approval – project has met all requirements
- Conditional approval – project requires further documentation or clarification
- Denial – Review Committee has made a reasonable effort to mitigate and resolve any outstanding or discrepant information required to approve the project. Reasons for denial will be communicated to the project lead.
Meaningful Participation Process:
- Project lead will provide a list of providers who had meaningful engagement in the project (ABMS Meaningful Participation Requirements).
- CPD Team sends an online attestation form to the providers.
- Provider completes the attestation form.
- Attestation information is submitted to the ABMS for credit (see the credit translation table for more information).
Project Requirements:
- Utilize OHSU’s A3 Problem solving format (include Problem, Current State, Root Causes, SMART Aim) [Example completed A3]. Check out a recent OHSU A3 Problem Solving webinar
- Collect pre-intervention baseline data
- Implement at least one test of change (i.e., PDSA cycle) designed to impact the Aim
- Collect post-intervention data to track and evaluate the test(s) of change
- Plot baseline data and follow-up data points on an annotated run chart
- Complete a brief attestation form confirming meaningful engagement in the QI project
