Emergency and Transitional Housing Application Submission Portal
Thank you for your interest in the Emergency and Transitional Housing Program. Please read this section thoroughly, as it contains important information regarding the application submission process.
- Complete the fields below to register your application.
- You must properly name each attachment as follows (Tip: Use the Checklist on page 2 of the application) :
- ETH Application PDF [Provider Name- Spring 2025]
- Be sure to type into the light blue text fields of the PDF application and attach that version. That is the only format we can process. The fillable application allows for electronic signatures; however, if the applicant prefers to print and sign the documents directly, you can submit the scanned signature pages as a separate file in addition to the saved “typed in” pdf application.
- Business License Documentation
- Housing Policies
- W-9 (signed and updated within the last twelve months)
- Vendor Profile (updated within the last twelve months
- Vendor Published Price Affidavit must be notarized
- 501c3 Letter
- Certificate of Commercial Liability Insurance must be current
- [Street Number and Name of Property] (upload the documents listed below as one PDF):
- Property Owner Verification
- Lease/Rental Agreement if applicable
- Zoning Documentation
- Additional Per Diem Request Budget
