This form should only be used if a concern about serious staff misconduct has already been raised with the Campus Principal or a Regional Principal and has not, in your opinion, been adequately addressed.

The serious misconduct must be a clear breach of the school’s Code of Conduct document that can be found on the school’s website. Should you have any questions, please contact the Regional Principal.
