To request your certificate, you must include a copy of a valid, signed photo ID.
Requests will not be processed without a valid ID.
Please allow 7-10 days for processing.
A certificate will be mailed to you. It cannot be emailed or faxed: the raised seal that authenticates the document does not transmit electronically. We are happy to mail directly to the church who is requesting your sacramental record.
If you cannot upload a copy of your ID, please print this form and mail with a copy of a valid, signed photo ID to: St. Hugo of the Hills, Attn: Sacramental Records, 2215 Opdyke Rd, Bloomfield Hills, MI 48304.
