You. Me. We. Virtual School Challenge

Thank you for registering for the You. Me. We. Virtual School Challenge! By completing the form below, you will be providing Abilities Centre Durham and Abilities Centre Ottawa with the information required for your class to join the Challenge. There are a few steps involved with completing this forms: 1. General School Information 2. Class Demographics 3. Student Permission Form 4. Teacher Attestation You will also receive an email on the Friday before each challenge week with access to the WebEx portal where the video resources will be stored. The You. Me. We. Virtual School Challenge team will provide instructions on how to log in and access the videos as well as instructions specific to the challenge at the end of the week. Our ultimate goal is to support you in creating excitement for physical activity within your classroom, and to highlight the variety of ways people can be active! We look forward to having your class join the challenge!

Student Permission Form

Please find below a link to download the You. Me. We. Virtual School Challenge Student Permission Form. The form attached is specific to photo/video consent. Completed forms will enable Abilities Centre to utilize photo and video submissions from teachers in the celebration video for each challenge. Ideally, the videos are shared with all schools to highlight the variety of ways we can participate in physical activity. Please note that signed permission forms are NOT required for general participation in the program, this is specific to the sharing of photos and videos. Teacher Attestation

As the teacher registering my class for the You. Me. We. Virtual School Challenge, I confirm that:

I will share the You. Me. We. Virtual School Challenge permission form with all students in my classroom and will collect signed permission forms for all students participating in the activity prior to the start of the first video.*

I, myself as the classroom teacher, and/or my fellow colleagues who are employees of my respective school board, will remain in the program space at all times and are responsible for the health and safety of all students while participating in the activity.*

You, Me, We Virtual School Challenge has been created to support safe physical activities in consideration of Public Health recommendations for physical distancing and limiting the sharing of equipment amongst students. I understand that, as the classroom teacher, it is my responsibility to enforce safe practices within my classroom and Abilities Centre Durham, Abilities Centre Ottawa and/or its partners are in no way responsible for any breech of the provincial recommendations within the classroom.*

• Any documents, photos, videos or recordings shared with Abilities Centre Durham and/or Abilities Centre Ottawa only contain images of students who’s parents have provided photo consent on the You. Me. We. Virtual School Challenge Parent Permission Form.*

I, myself, grant and release to Abilities Centre Durham and Abilities Centre Ottawa the right to use photographs, audio tapes and/or videotapes in which my students appear in any information, promotional and/or marketing materials such as videos, films, recordings, still photographs, digital images, social media (Facebook, Twitter, You Tube, Instagram, etc.) or articles relating to Abilities Centre Durham and Abilities Centre Ottawa, its programs and services including but not limited to, You. Me. We. Virtual School Challenge social media postings, membership brochures, newsletters, annual reports or our website, whether broadcast on television, radio or any other medium. In the event that these images are to be used for any other purpose by Abilities Centre that is inconsistent with the purposes outlined above, it is understood and agreed that my prior consent shall be obtained.
