IEEE Preferred Registration Providers Access Request Form

This form enables users to request access to an IEEE preferred registration system. It collects key details to ensure access is granted according to the user’s role. All submitted information will be reviewed by system administrators.

Third-Party Event Management Providers: Please ensure that a fully executed statement of work is on file with IEEE for the relevant event(s) before requesting user access. Access will only be granted upon verification of a fully executed agreement.

Organization/Company Name

Enter the name of your organization or company, if applicable. This helps us verify your role and the need for access.

Full Name (First and Last Name)

Enter your full name. This will be used to identify you within the registration system.

Email Address

Enter a valid email address. This will be used for communication regarding your access request.

IEEE Conf. ID (If applicable and known)

Preferred User Name

Choose a unique username you wish to use for accessing the registration system (Note: This may be subject to availability).

Access Start Date

Specify the date you need access to begin. This will help us schedule your access appropriately.

By submitting this request for access, you agree to the following:

Authorized Use Only: Access to the registration system is granted solely for the purpose of performing your designated responsibilities in relation to the event(s) listed in your request. Unauthorized use of the system or sharing access credentials with others, is strictly prohibited.

Confidentiality: You acknowledge that any data accessed through the registration system may be confidential and protected by privacy laws. You agree not to disclose, share, or use this data outside the scope of your assigned responsibilities. This includes personal information of event participants, volunteers, and speakers.

System Integrity: You agree not to engage in any activity that could disrupt or impair the system’s functionality, including but not limited to unauthorized attempts to access restricted areas, data corruption, or tampering with system files.

Account Security: You are responsible for maintaining the security of your account, including keeping your login credentials confidential. Any unauthorized access to your account should be reported immediately to the system administrators at cee-services@ieee.org.

Termination of Access: Access will be revoked at the discretion of the system administrators if any of the terms outlined in this agreement are violated.

Limitation of Liability: The system administrators are not liable for any loss or damage arising from your mis-use of the registration system.
