Fill out the workbook in its entirety and upload in the space provided below. Measure selections will not be accepted if the workbook contains any errors. Please review the Overview Page of the workbook before submitting. If assistance is needed to complete the workbook, please contact HCPF_primarycarepaymentreform@hcpf.state.co.us.

Attestation Statements

On behalf of my organization, I have reviewed the measures selected and understand that the organization will be responsible for meeting, or showing marked improvement, on performance metrics for the measures selected in order to earn APM quality points.


A total of 200 APM quality points (100 points for PCMHs) is required to continue to receive the highest available Medicaid payment rate. PCMPs with an APM Quality Score of fewer than 200 total points will receive a reduced payment rate.


I understand that submissions with errors will not be accepted without revision. If a measure selection workbook that is free of error is not received, HCPF may automatically assign default measures to the PCMP.
