Reporting Submission Form
Please complete the SHIP - Change Order Reporting Form and attach it with all necessary supporting documentation to this submission form. Travis County shall be informed of all changes so the County may confirm all proposed change are consistent with the intent of the subaward contract provisions. COs are between the Subrecipient and the General Contractor. This reporting requirement allows Travis County to confirm subrecipient contract compliance.
