Field Trips Permission Slip Forms

HSI-SL is providing an excellent opportunity to take Freshmen, Sophomore and Junior students on a field trip to SeaWorld San Antonio on Friday, May 19th, 2023 from 6:30 am to 8:30 pm and Moody Gardens on Monday, May 22th, 2023 from 8:00 am to 4:00 pm. Students will get a day to spend and explore educational activities. Students will be able to enjoy the Physics of SeaWorld park rides and study the human body systems and the effect of adrenaline.

We encourage you to send your child on an experience they will enjoy and learn at the same time! Permission slips are due by 1st of May and the Due day to pay for the Field trip is 1st May in order for your child to be able to attend the Junior Trip.

IMPORTANT NOTES:

Please make sure to submit this form ONCE.
No submissions will be accepted AFTER the deadline. It is the students'/parents/guardians` responsibility to ensure that all required information is provided and the permission slip forms are complete.
Please do NOT send any forms via emails. The requests that are sent by emails or handed to any school staff will NOT be considered.

Grade Level Field Trips

09th Grade: Monday, May 22, 2023 (Moody Gardens, 8:00 am. - 4:00 pm.)
10th Grade: Friday, May 19, 2023 (SeaWorld, 6:30 am. - 7:00 pm.)
11th Grade: Friday, May 19, 2023 (SeaWorld, 6:30 am. - 7:00 pm.)

The total cost for these events can be found below

Moody Gardens: $15 (Per Person)
SeaWorld: $50 (Per Person)

which will cover all transportation and activities (food is not included).

Please be sure to upload the permission slip to this form. Payments will be made online on Revtrack (Ask at the front office for more details).

Spaces will be on a first-come, first-served basis so sign up as soon as possible to reserve your spot.
