This form may only be completed if the Take -Action project had an Approved Proposal from the Silver Award Committee AND has completed that said project.

Each girl from the Silver Take-Action project must submit an individual form once the project is completed. One submission for an entire troop will not be accepted. If possible, please try to have the Girl scouts submit their entry on the same day.

Note: The maximum number of girls per project that can earn the same Take-Action project award is 4, OR the Project will be denied.

*To earn a Silver Award, the project must be completed by September 30th after finishing 8th grade.

*To participate in the High Awards Ceremony, the Silver Take-Action report must be completed by December 31st before ceremony date.
