INMATE MEDICAL RECORDS REQUEST

Inmate medical and mental health records are not released as public records. Please follow the guidelines outlined in Policy OP-140108 when submitting a medical or mental health record request that includes Protected Health Information.


A valid Authorization for Release of Protected Health Information form must be submitted with your request if it is not already on file. ODOC may refuse to honor a written authorization to disclose PHI from the inmate, other persons, or outside organizations for reasons outlined in the above policy. Note: separate releases are needed for medical and mental health information.


PHI may be disclosed without authorization when specific minimum standards are met with public health authorities, law enforcement, and other officials as outlined in the above policy, for purposes such as health care treatment and operations, audits, investigations, inspections, licensing or disciplinary actions, and civil rights compliance.


Medical requests may be subject to fees for the costs of record searches and copies per the fee schedule.

  • $10 search fee.
  • $0.30 per page with a $200 cap.


DO NOT send money before receiving notification of applicable fees and the exact amount due.

Please specify the nature of your request and the records you seek, providing detailed information to make the search as efficient and timely as possible. Include the inmate's full name, ODOC number (if known), date of birth (if known), and/or any applicable time frames.


Please also be advised that the Open Records Act does not require ODOC to create a record not otherwise in its possession.

I acknowledge that if the records are available in electronic form, they may be provided to me by email or other electronic means. I further acknowledge there might be a search fee for records, a fee for copies of records if I prefer to receive the records as paper copies or on a disk or thumb drive, and a fee if I request certified copies. I also may request to inspect the records in person instead of being provided with a copy.
