This disclaimer of Liability and Release of claims is to be executed by the participant, or if the participant is a minor, by the participant’s parent/guardian. The application for program registration (“Application”) will not be accepted unless it has been executed. In consider of the Lefroy Skating Club, accepting this application I agree to the disclaimer of liability and release of claims. Disclaimer: The participant assumes all risks associated with his or her participation on the programs offered by the Lefroy Skating Club. The Lefroy Skating Club accepts no liability for bodily injury, death, property or loss due to any cause whatsoever, including, without limitation, negligence on the part of the Lefroy Skating Club, including its elected officials, employees and agents. Release: The participant and his or her parents/guardians waive any and all claims they may now and in the future may have against, and release from all liability and agree not to sue, the Lefroy Skating Club, and its elected officials, employees/coaches and agents. This release includes all claims for bodily injury, death, property or loss sustained by the participant as a result of his or her participation in the programs offered by the Lefroy Skating Club, including, without limitation, negligence on the part of the Lefroy Skating Club, its elected officials, employees/coaches and agents. By signing this application, I certify that the applicant is in good physical and mental health. In the event of an inability to contact me, I herby give the Lefroy Skating Club, permission to seek out any necessary medical assistance that my child may require while attending this program. I herby give permission for Lefroy Skating Clubs, staff to take photographs of my child, children or myself. I understand that the photographs may appear in on the Lefroy Skating Clubs, bulletin boards or advertising and/or Lefroy Skating clubs web site or social media. Refunds: There are no refunds. Covid 19 Disclaimer: Please note that, given the ever-changing response required to diligently and properly address COVID-19 concerns, the Lefroy programs are subject to change with little to no notice. Lefroy Skating Club reserves the right to cancel or alter ice times or programs, as required. Compliance with protocols put into place is a condition of your in good standing registration. I confirm that I have read this agreement before signing it and that I understand it and that it is binding not only the participant and me but also on our helpers, executors and assigns.
