Transfer Scheduling Request

Name of person requesting the transfer

Include the approved Proposal for the course/event you would like to schedule. Proposals are good for 3-5 years once they are approved.

Whole-body, preserved, undissected donors requesting to be delivered to your institution.

Please include desired pathology, contraindicated pathology, as well as desired gender mix. We will do our best to accommodate your requests.

Please list any requested anatomical specimens (i.e. brain, joint, etc.). We will do out best to fulfill your request. If provided, the specimen(s) will be added to your school's Long-Term Loan. Specimens used for Long-Term Loans carry a 5 year time limit from date of procurement. Dates will be communicated with you by ABP Staff.

Whole-body, preserved, dissected donors being transferred back to Anatomy Bequest Program

List any additional anatomical specimens (i.e. brain, joint, etc.) you are returning to the Anatomy Bequest Program. Include donor acquisition numbers, as well as specimen laterality as applicable. You may include a list here or upload a file below.

Who will provide transportation for the donors?

Update as necessary to include reference numbers, POs, or chartstrings, or contact information. Please review your existing proposal to ensure accurate accounting information. Leave blank if there are no changes. Inaccurate/outdated information may result in a delay in our ability to support this event or lead to incorrect invoicing.
