Request for Return to Active Status

To reactivate an inactive program, the chief executive officer or an officially designated representative of the program sponsor must authorize and submit the following Request for Return to Active Status form.


Upon review of the submission, the program sponsor will be notified by the CoA of additional requirements, if any, that must be met to restore active status. CAAHEP will return the program to active status upon consultation with the CoA.


Program sponsors that fail to notify CAAHEP and the CoA of its intent to reactivate the program by way of the Request for Active Status form, may be recommended that accreditation be withdrawn to the CAAHEP Board of Directors by the CoA.


If the program sponsor choses to not reactivate the program, a Request for Voluntary Withdraw of Accreditation will need to be submitted.


If the program sponsor requests additional time on inactive status, contact the CoA for approval. CAAHEP will update an inactive status deadline upon consultation with the CoA.
