Make sure that you provide accurate details for all information requested. Failure to complete this form in a timely or accurate manner violates the N.U.in/Global Scholars/London Scholars Supplemental Code of Conduct and may result in disciplinary action.

In addition to completing this form, you must complete the Northeastern University Travel Registry. Travel to high risk destinations is prohibited unless it meets one of the criteria outlined in the Personal Travel form. For a list of high risk destinations, please review the High Risk Country List. Semester In students are also required to complete the Personal Travel Form. Please make sure to provide accurate details for all requested information; Failure to do so may result in disciplinary action.
