Licensing Options

Standard: Integrate a subset of our advance care planning materials (i.e., licensed advance directives and pamphlets) into your workflow and technology platforms. Option to co-brand with your institution's logo and contact information.

Powered by PREPARE: Customize, white label, and integrate a complete suite of our advance care planning materials into your workflow and technology platforms.

Research and Quality Improvement (QI): Conduct research or quality improvement initiatives using our advance care planning materials.
Student: For students or trainees only. Allows for use of PREPARE URL in materials and printing unmodified PREPARE materials for specific time-limited use. $500 minimum.
Basic: Allows for use of PREPARE URL in materials and printing unmodified PREPARE materials for specific time-limited use. $10,000 minimum.
Premium: Basic, plus an initial set-up meeting with the PREPARE team, aggregate usage data, and a custom URL. $20,000 minimum.
