Safety Issue Submission

Please use the following submission form to report a safety concern.


  • If you are reporting anonymously, you will be provided with a tracking number to view the status of your report on the public dashboard.
  • Users can view the status of their report or update report details by visiting the safety concern portal and logging in with the same email address you used for your report.
  • For simplicity, it's recommended to select the Microsoft authentication option to utilize the campus Single Sign On feature.
  • You can also create a free SmartSheet account using the same email address and your own password.

Important considerations for anonymous reports:

  1. Anonymous reports limit our ability to contact you for additional details.
  2. Additionally, you will not see this report listed in your safety concern portal which will prevent you from viewing your submission history or being able to update the details of your report.
  3. You will still receive a tracking number to view the status of your report on the public dashboard.
