The purpose of the occurrence/accident investigation(s) is to ascertain the sequence of events, contributing factors, determine corrective actions and monitor the completion of corrective actions.
-- All sections of the report must be completed.
-- Please allow up to 72 hours for initial contact.
-- If this is an Emergency please call 911.

Copy email so that you don't have to retype it below in order to recieve confirmaiton of this report for your file.

--Please write a first-person, chronological narrative of what happened when you reported to the scene. Use the full names of each person included in the report.
--Start a new paragraph to describe each person's actions separately. Answer the who, what, when, where, and why of what happened. Write as much as you can remember - the more details, the better.
--Don't leave room for people reading the report to interpret something incorrectly. Don't worry about your report being too long or wordy. The important thing is to report a complete picture of what occurred.

Attach supporting documents if applicable (emails, photos, written materials, etc.). If at all possible please include pictures with your report
