Thank you for new or continued interest as a partner with Delta Flight Products (DFP) to provide the highest quality parts and services in the aviation/aerospace community. You are being requested to complete this self-audit quality survey as part of DFP’s commitment to our customers to provide high quality products. NOTE: Please allocate enough time to thoroughly complete each applicable section of the survey that pertains to your organization. This form cannot be saved and restarted at a later time. Ensure that copies of certificates (AS9100, ISO9001, etc.) are available to attach to the form. Please review the instructions below and thoroughly complete each applicable section of the survey that pertains to your organization. The applicable section will display when you select the "Type of Supplier" appropriate to your business. You can select more than one. Front Matter – This section must be completed by all suppliers and service providers. Section A – for manufacturing, fabrication, machining, repair or assembly operation for DFP. This section may be omitted if your organization is AS9100 or ISO9001 certified. Section B – for any special processing. Section C – for a distributor or pass-through distributor. Section D – for any calibration services. Section E – for any Engineering Services or Testing Services. Section F – for Raw Material providers. Section G – For certified Repair Stations (14 CFR 145). By completing and submitting this survey you and your organization agree to abide by any Terms & Conditions of the PO, Quality Clauses, and non-disclosure agreements in place with DFP. Also, by submitting this survey your organization agrees that an on-site audit may be conducted, at DFP’s discretion, to verify the accuracy of any information provided.
