Please complete this form to request to change information related to your graduation status or commencement ceremony preferences. Using this form you may request the following: *Change your intended graduation term *Update your diploma mailing address *Update your commencement ceremony attendance preference *Request special accommodations at the commencement ceremony *Ask other questions To view your Graduation Application and current preferences in BanWeb, please log in to MyUWG, select the BanWeb tab, select the "Student Services & Financial Aid" link, and then, select the "Graduation Application" option. At the bottom of the page, select the "View Active Graduation Applications" link. If you have any trouble using this form, please contact us at graduation@westga.edu.

Please input the course(s) for which you were unable to register, that you failed, were withdrawn, or where you received an incomplete grade. Courses should be listed as "SOCI 1101". Please separate multiple courses with a comma "SOCI 1101, ENGL 2103".

Use this form to update your diploma mailing address. Students should expect to receive their diploma within 6-8 weeks of graduation. Students must complete this form and update their diploma mailing address prior to their official graduation date. Diplomas will be mailed to the address indicated on your Graduation Application. After this request is processed, you will be able to see the change to your diploma mailing address on your Graduation Application in BanWeb.

If you feel that you need special accommodations at the Commencement Ceremony, please let us know how we're able to assist you by completing the form below. You must complete this form even if you have accommodations on file with Accessibility Services. You do not have to identify why you need special accommodations, rather, please specify how we're able to assist you. Graduation is your day - we want to do whatever we can to make it special for you. Common reasons for using this form: *Unable to stand for up to an hour prior to the start of the Commencement Ceremony *Use of a service animal during the ceremony *Mobility issues that will prevent you from being able to walk in a line at a typical cadence Please provide details on how we're able to assist.
