Grant Overview

The goal of the RISE Grant Program is to measurably improve the quality of and access to medical and behavioral health care for the GCHP Medi-Cal population in Ventura County.


The four strategic priorities for the grant program include:


  1. Improve access and connections to care for GCHP member populations and/or geographic areas of Ventura County where there are objective (statistically significant) unmet healthcare needs or access issues (e.g., pediatric care, where the average wait time for access exceeds objectively reasonable time periods).
  2. Bring care to GCHP members where they live, work, and/or go to school, through home-based care, community-based care, school-based care and worksite-based care, or otherwise addressing how care is organized or grouped for ease of use.
  3. Improve GCHP member health outcomes, experience, and education, including update of benefits and services that are culturally responsive and focused on health equity as measured by the State or Federal funding sources.
  4. Offer other alternative and/or non-traditional healthcare solutions or modalities intended to remove structural barriers to care, reduce healthcare costs, and/or otherwise improve access and/or efficiency concerns, including but not limited to transportation, mobile clinics, telemedicine and other technology driven initiatives.


The overall three-year grant program runs from July 1, 2025 to June 30, 2028, and the initial grant period will be July 1, 2025 through June 30, 2026. Applicants may apply for multi-year grants, but preference will be given to 12-month grants in the initial grant period.


Awards will be made for the grant period, and funds will be distributed to grantees annually on a quarterly payment schedule, provided the grantee has shown satisfactory performance and is in compliance with the conditions of the grant award.

The Institute for Healthcare Improvement (IHI) will host two informational webinars and two open office hours sessions to assist prospective grant applicants with application drafting and review.


These offerings are optional and intended to support prospective applicants with submitting a robust and competitive application. Dates for the calls are:

Describe your project aim, including how the project will measurably improve either access or connections to care, bring care to GCHP members where they live, work, and/or go to school, improve GCHP member health outcomes, experience, and education, or remove structural barriers to care, improve quality of care, reduce healthcare costs, and/or otherwise improve access and/or efficiency concerns for Medi-Cal members. The statements should be well-written and clearly describe what you intend to accomplish in SMARTIE (SPECIFIC, MEASURABLE, ACHIEVABLE, REALISTIC, TIMELY, INCLUSIVE AND EQUITABLE) terms. (150 words)

Provide an assessment of the problem that your project is intending to address. Clearly articulate how your project relates to one of the four strategic priorities of the grant program. Include data that demonstrates unmet health needs or access issues in this area. Describe your organization’s efforts to date related to the project problem/background. (300 words)

Described the anticipated initiatives or activities of your grant project, including the specific population(s) of focus (e.g. geographic areas, sociodemographic groups, etc.). Include any preliminary data to support your project design, hypotheses and anticipated outcomes. A driver diagram is optional but encouraged (see appendix above). Describe how the initiatives or activities will be sustained after the grant period ends.

Provide a detailed overview of the project timeline and major milestones/deliverables over the course of the grant period. Describe if you intend to apply for a 12-month or multi-year grant. Your milestones should align with your aims/goals and project plan and be reasonable in expectations. Describe if your project requires an activation or start-up period and include a timeline and milestone for the activation phase.


(note: activation periods should be six months or less in duration and should clearly relate to the new programs or initiatives described in your project design).

Describe your measurement strategy for the grant project (you are strongly encouraged to utilize the Measurement Strategy Template and upload below), including plans for data collection – type of data to be collected, by whom, how often, how the data measures your intended project aims – and data analysis.


Please identify at least one access, health outcomes/experience/education, or cost/utilization/efficiency measure that your project is intended to improve. It is recommended to include at least two process measures and one outcome measure in your measurement strategy (see appendix above for references).(300 words)

Include a list of all project team members, their anticipated role(s) on the project, their contact information, and a 100-word bio. Roles might include: team lead, senior sponsor, data collection lead, fiscal lead, etc.
