Portland Public Schools PTHV Extended Pay Request (training & home visits)

Portland Public Schools PAT-represented educators may use this form to request extended pay for PTHV training and home visits if the activity occurred outside of your regular work day.

Please submit a separate form for each activity in which you are eligible for extended pay. For example, if you did two home visits, one at 5:00pm and the other at 6:30pm, please fill out a separate form for each visit.

EXTENDED PAY (please read instructions)

Portland Public Schools PAT-represented educators are eligible for extended pay for PTHV training and home visits if they occur outside of the regular work day.

The data collected on this form will be compiled, verified and submitted directly to payroll for extended pay processing. You do not need to submit an extended pay request to your school. You can select to have a copy of your request emailed to you for your records.

Extended pay requests MUST BE RECEIVED BY 5:00PM ON THE 15TH OF EACH MONTH in order to be included in the current month's paycheck.

PLEASE NOTE that your eligibility to receive extended pay will be verified either through a PTHV training attendance roster or a Home Visit Reflection entry. If eligibility cannot be verified by the submission deadline, the extended pay request will roll over into the next pay period.
