Nominations are due by 11:59 p.m. December 3, 2023.

OHSU alumni and community members are encouraged to submit nominations, which are reviewed and selected by the OHSU School of Nursing Alumni Association’s Awards Committee and endorsed by the School of Nursing Alumni Advisory Council. Nominations remain active for three years.

Please note that this form cannot be saved and returned to once started. Before beginning the form, please prepare a single .pdf file containing: the nominating statement; nominee CV; and up to two letters of support. Nominating statements and any optional supporting letters should be no more than 750 words in length.
