My signature below indicates that I have read the guidelines and understand that:
A. The Parks & Recreation Department in cooperation with the City Arborist will make all final decisions, including but not limited to availability, location, style, and species.
B. Donations must meet a need of the property; not interfere with the intended current or future use of the property; and not require relocation of equipment or infrastructure to accommodate the donation.
C. Donated features will be ordered and installed after they have been paid in full. Payment must be received prior to installation beginning.
D. The Parks & Recreation Department shall verify that the name of an honoree requested to appear on a plaque or paver belongs to an individual, whether living or deceased. The Department may require an applicant provide any additional information reasonably needed to verify this information.
E. The installation timeline will be set by the City based on availability, delivery window and staffing.
F. As a tax deductible donation, all features are the property of the City of North Port and will be maintained for the expected life cycle of the donation.
G. If during the donation period a monument is damaged beyond repair due to an act of God, vandalism or other unforeseen circumstance, the Department shall replace the monument one (1) time with a similar monument, if available. There is no obligation to replace damaged monuments more than once during the donation period. There is no obligation to maintain, repair or replace a monument following expiration of the donation period.
H. At the end of the donation period, the donated feature will be available for renewal with the first right of refusal given to the original donor. Pavers may remain in place for the length of it's useful life, as determined by the Parks & Recreation Department.
