Potential Partnership Inquiry Form

iS CLINICAL partners with licensed clinical skincare professionals including medical spas, plastic surgeons, dermatologists, and high-end luxury and clinical skincare facilities. We require proof of the proper professional clinical license, business zoning, and resale tax licenses needed in your state or region to become an authorized iS CLINICAL partner. To apply to become an iS CLINICAL authorized partner, please complete the below questionnaire for consideration. Upon completing this questionnaire, and if you meet the qualifications to become an iS CLINICAL partner, a company representative will contact you.

Is your current business location where you practice zoned for medical and/or skin care services?*

(Medical oversight includes an MD, DO, NP, or PA license working within and insuring your business)

U.S. iS CLINICAL Partners are authorized to sell within the US territories, and are not authorized to discount iS CLINICAL products online, or sell through third parties. Do you understand this policy?*
Do you have a copy of your valid state's resale certificate?*

Required for all states except DE, AK, MT, NH, & OR.

A resellers permit is required to become an iS CLINICAL Partner. iS CLINICAL products are considered retail products and therefore must follow state IRS requirements to collect resale taxes. If you do not possess a current Resale Certificate, we cannot open you as an iS CLINICAL Partner until you obtain one. Do you understand this policy?*
