PLEASE NOTE: Completing, signing, and submitting this application constitutes your permission for us to reach out to your vet. Both parties agree that this application can be executed by the applicant and sent to True North GSD Rescue via this online form. If the applicant would like to submit this document via this online form, applicant and True North GSD Rescue both agree that the applicant’s signature is valid and binding upon the parties. If you would like to submit an application by US Postal Service, please email info@truenorthgsd.com requesting a printable application.

I certify that I am at least 18 years of age and that any misrepresentation of the true facts in this surrender application agreement will invalidate the surrender agreement and will give True North GSD Rescue the legal right to terminate any agreements immediately. Upon surrender of the dog to True North GSD Rescue, I will relinquish all ownership of the dog to the rescue. I will provide all documentation regarding breeder to the rescue, if applicable. True North GSD Rescue will confirm breeder return requirements prior to the rescue taking possession of the dog. If the dog was received from a breeder, and the breeders contract requires the dog be returned, I understand the True North GSD Rescue will not accept the dog into the rescue.

I understand they will also evaluate the dog and will accept the dog into the rescue based on the evaluation and space. I am in full agreement of the terms of the surrender. True North GSD Rescue shall not be liable for, and is hereby relieved from, all liability for any damage, expenses, causes of action, fines, suits, demands, judgments, or claims of any nature whatsoever arising from or by any reason of any damage to property or injury to any persons caused in whole or in part during the visitation of the dog up to surrender.

Upon receipt of any dog that we have agreed to take, that we have completed due diligence in reference to, True North GSD Rescue accepts the dog and agrees to take on all liability from the date of surrender, not to include any liability prior to the date and time of surrender.

So, if you sign the form but we have not yet taken possession of the dog, then the dog, the dog’s actions and the dog’s expenses remain the responsibility of the owner.

I hereby accept and assume such liability and agree to protect, indemnify, and hold True North GSD Rescue and all representatives, agents and volunteers harmless from and against all of the aforesaid.
