Virtual Advisor Information Form

Thank you for your interest in the Phoenix Children’s Virtual Advisory Community! Virtual advisors are an important part of Phoenix Children's mission to advance hope, healing and the best healthcare for children and their families. Advisors receive up to two short surveys per month via email. Some surveys may be regarding general experiences. For example, food service offerings in our Thomas campus cafeteria. Other surveys may be based on your specific experiences in the health system. For example, the parent of a child who sees Phoenix Children's heart center providers, may receive a survey about potential signage in the Heart Center. Virtual advisors' feedback will have positive benefits on patient care!

By filling out this information form, you will help us better tailor the surveys we send you to your experiences at Phoenix Children's. We strive to ensure our Virtual Advisory Community has wide representation and reflects our patient population. The information collected on this form will be kept confidential and used only in relation to the Virtual Advisory Community.
