The following documentation must be submitted with the Request for Enrollment into the CEWRI-HH Renewable Energy Installation Company Roster:
- Renewable Energy Installation Company Enrollment Submittal Form (filled and completed);
- A Commitment of Compliance with CEWRI-HH Terms and Conditions Sworn Statement;
- Evidence of the company’s registration with the Puerto Rico Department of Consumer Affairs (DACO for its Spanish acronym);
- Evidence of at least one (1) company employee being a Certified Installer under Regulation No. 7796, or its subsequent equivalent regulation;
- Evidence of the company’s enrollment in the Federal Government’s System of Award Management (SAM) (https://sam.gov/content/home);
- A Certificate of Good Standing, valid at the time of submission, from the Puerto Rico Department of State in the name of the company;
- A Debt Certificate (“Certificación de Deuda”), valid at the time of submission, issued by the Puerto Rico Department of Treasury in the name of the company;
- A Certificate of Income Tax Returns (“Certificación de Radicación de Planillas”), valid at the time of submission, issued by the Puerto Rico Department of Treasury in the name of the company; and
- An Account Statement (“Estado de Cuenta”), valid a the time of submission, issued by the Municipal Revenue Collection Center (CRIM for its Spanish acronym) in the name of the company.
- Evidence of Public Liability Insurance to cover damages ranging no less than $1,000,000 by occurrence and $1,000,000 by aggregate for the installations to be performed for CEWRI-HH eligible Applicants.
- A Renovation Firm Certificate, valid at the time of submission, issued by the Environmental Protection Agency in the name of the company.
