The following documentation must be submitted with the Request for Enrollment into the CEWRI-HH Renewable Energy Installation Company Roster:

Renewable Energy Installation Company Enrollment Submittal Form (filled and completed);
A Commitment of Compliance with CEWRI-HH Terms and Conditions Sworn Statement;
Evidence of the company’s registration with the Puerto Rico Department of Consumer Affairs (DACO for its Spanish acronym);
Evidence of at least one (1) company employee being a Certified Installer under Regulation No. 7796, or its subsequent equivalent regulation;
Evidence of the company’s enrollment in the Federal Government’s System of Award Management (SAM) (https://sam.gov/content/home);
A Certificate of Good Standing, valid at the time of submission, from the Puerto Rico Department of State in the name of the company;
A Debt Certificate (“Certificación de Deuda”), valid at the time of submission, issued by the Puerto Rico Department of Treasury in the name of the company;
A Certificate of Income Tax Returns (“Certificación de Radicación de Planillas”), valid at the time of submission, issued by the Puerto Rico Department of Treasury in the name of the company; and
An Account Statement (“Estado de Cuenta”), valid a the time of submission, issued by the Municipal Revenue Collection Center (CRIM for its Spanish acronym) in the name of the company.
Evidence of Public Liability Insurance to cover damages ranging no less than $1,000,000 by occurrence and $1,000,000 by aggregate for the installations to be performed for CEWRI-HH eligible Applicants.
A Renovation Firm Certificate, valid at the time of submission, issued by the Environmental Protection Agency in the name of the company.
