PSC PAC Prior Approval Form

Federal law requires members to sign this prior approval form for the executive or administrative personnel of the member corporation to be solicited by PSC PAC. The Company may not approve a solicitation by any other trade association for these years.


This form may designate a single individual or wider group of eligible contributors to be solicited based on the signer’s selection.
