Recently completed an online SWS training; or
Failed your SWS exam after your in-person SWS training

Springfield/Pendleton Exam Location: Once your request form is submitted someone from DWS will review the information and you will receive an email from forms@app.smartsheet.com with a PDF of your exam confirmation.

Portland Exam Location: Select the date you want to take the exam, and include a time. DWS will review the request, if we can accommodate your date/time you will receive an email from forms@app.smartsheet.com with a PDF of your exam confirmation. If we aren't able to accommodate your date/time we will contact you by phone/email to set a date/time.
