COVID-19 Contact Identification and Notification During Summer - Training Registration Form

This training is for summer school personnel and staff who will support and provide contact identification and notification when a COVID-19 case or outbreak occurs. This role works in coordination with an LPHA when an LPHA is without the capacity to meet school or district needs. If this training is right for you, please complete this registration form. You will receive a follow-up email to register for the training in Zoom. After registering, for the training, you will receive a second email with the link to the training, which will be held in Zoom.
